A Code 05 Industry-Sponsored Project is one of the only work-based learning experiences a student can complete without a job, a car, or a placement slot that doesn't exist in their community. The employer brings a real, non-urgent problem. The student works it. The mentor meets them twice.
The barrier was never the idea, it's the paperwork: what the training plan says, what hours count, what gets retained, and what you're actually asking the employer to give. Every packet answers all four before you make the first call, and every hour log is built to sum to the 60-hour Sustain Fund threshold, because hours documented wrong the first time are dollars your district doesn't recover.
Whether you choose to use one of our Industry Project kits or not, CDE requires you to complete this form prior to implementing any project where you'll be claiming state or federal funding or counting student hours towards official state metrics.
